eReader - keep your books when you change device

Ok, I am not condoning piracy, but when you buy an eBook for your current eReader and a new one comes along in a year to two which you covet, you should be able to transfer the books which you purchased with hard earned cash to the new device.  Well, at present you cannot unless you know someone who can help.  Until now!

Here are some links to a helpful discussion with detailed instruction how to rid your newly purchased books of DRM:

Hopefully this will be a boon for you all.

Sorting Out Cholesterol and Coronary Artery Disease — NEJM

Sorting Out Cholesterol and Coronary Artery Disease — NEJM

Well, well. A genetic defect which may explain high LDL/VLDL levels in Europeans (up to 68% of us carry this gene). Carriage of the gene confers a 29% excess risk per allele for coronary artery disease. This news cannot help me now, but offers hope for many folks with (familial) LDL/VLDL hyperlipidaemia in future with the anticipated development of novel agents to target the C/EBP–sortilin 1 pathway.

Investigation update...

Excellent news received this week - the horrible stuff in my right lung appears to be nearly gone for some unknown reason.  The CT scan in October has finally been reported and compared with those from London hospital.  This is fantastic news.  Now, my fingers are crossed that this means there is a lower risk for me suffering a serious lung infection this Winter.

And what about my heart?  Echocardiogram and treadmill assessment demonstrated that heart is ... virtually back to normal!

Holiday Season and gift giving

This afternoon one of the booked patients arrived with her husband.  "Here you are," he said handing over a bottle of wine in a nice bag.  "For all you have done for my wife this year we want to say thank you."

Well it would be impolite not accept the gift, but this couple are not well off, and my care of the lady is a professional responsibility which is taken seriously.  Lovely that the care is not taken for granted, but why does the couple feel the need to offer a gift of gratitude?

Retinal-Vein Occlusion — NEJM

Retinal-Vein Occlusion — NEJM An excellent review of a common cause of visual loss. Subscription needed to view full text (at least until April 2011)

GP Commissioners need to be aware that this problem affects nearly 2% over 40's (10 year incidence is 1.6%).

Biologicals can be used to maintain vision if central vein occlusion with macula oedema (as many as 20% cases), and the treatment needs to be given as often as monthly in some cases.

Responding to an FDA Warning — Geographic Variation in the Use of Rosiglitazone — NEJM

Responding to an FDA Warning — Geographic Variation in the Use of Rosiglitazone — NEJM A good article showing how crazy USA healthcare drug regulation can be. A drug which is banned in Europe now, yet can still be prescribed in USA. There is a clear increased risk of cardiovascular disease in diabetes mellitus patients taking this drug.

If you know a relative or friend who is still taking rosiglitazone, please, please get them to see their doctor to change to an alternative (e.g. pioglitazone) or a different agent altogether.

Doing this may save a life or two, and reduce healthcare costs overall!

FDA approves Pradaxa to prevent stroke in people with atrial fibrillation

FDA approves Pradaxa to prevent stroke in people with atrial fibrillation may spell the death knell for the upiquitous INR/anticoagulation clinics. If not pradaxa, perhaps a future agent. The hidden costs of transport, staffing, interpretation, and informing patients which are generated by INR clinics may be a good reason to encourage drugs like pradaxa. In the 21st century, INR clinics are an anachronism, IMHO.

Review time...

Great news, almost 3 years since the heart attack which changed my life for ever.

Echocardiogram today was normal.

On the treadmill, I managed 9 minutes to complete stage III with only 5 minutes needed for my blood pressure and pulse to recover to resting levels. The ECG remained OK throughout!

My karate training has resumed - I managed 1 hour of training last weekend.

My weight is the main concern - but after all I have suffered, why shouldn't I enjoy my food?

Wow, it is a while since my last post :-( Guess this is due to getting very busy with full time working. Psychotherapy finished September 2009. Have just returned from a month abroad (return delayed by the Iceland volcano's ash cloud).

Really do need to get going with exercise :D
